Please return this form with your payment and mail to the PO Box listed above.  Please make checks payable to Christine Smith.  You will receive a confirmation by e-mail.  If you mail in money and do not hear from us, please e-mail Christine (Davis) Smith at chris-bill@sbcglobal.net.  No tickets will be sent to you. Your name will appear on a guest list at the registration table.  Your canceled check will serve as your confirmation.  Money is non-refundable after August 23, 2009.  All refund requests must be in writing.

There will be a $25.00 service charge on all returned checks.
